To be eligible for a Community Respite Service membership, according to CRS’ by-laws you must be:

• 18 years or older and have a physical or intellectual disability.  OR

• An adult care provider of an individual with an intellectual or physical disability.

If you would like to become a member of Community Respite Service for this fiscal year (April 1-March 31), please print the Membership Form and mail along with the $25 membership fee to:
